4. Issuance of Medical Certificates

Medical certificates required for career development activities can be issued based on medical checkup results. Please use the automatic certificate-issuing machine or visit the office of the Center for Health Service Science (Health Service Center) with your Student ID Card. Requests via phone and mail cannot be accepted.

Please note that the Center for Health Service Science cannot issue medical certificates if you failed to receive a regular medical checkup or if you did not consent to receiving a chest X-ray examination.
